Patient Medical Records Cowlitz 2 Fire & Rescue takes the security of your medical record(s) and the private information seriously. It is our business to make sure that your protected health information is maintained responsibly. Medical records are not public information as fire reports are, even though C2FR, a public entity responded to and treated the patient. Many persons are unaware of this. To obtain a copy of your medical record you may get it at the our main station at 701 Vine St, Kelso WA 98626; or call 360.578.5218. You will be required to complete a Release of Medical Information form and show photo identification. The person that may assist you with obtaining your medical record is the Heath Insurance Portability and Accountability Act (HIPAA) Privacy Officer, Becky Ribelin, the EMS Administrator. You may also view and/or copy our Notice of Privacy Practices to ensure that we are compliant with federal law to keep your medical record private. Each patient receives this notice and is required to sign that they have received it. |
